We accept Medi-Cal, Medicare, CMSP, Path to Health, and Partnership Health Plan. While we don’t accept private insurance except for urgent care visits, we will bill your insurance as a courtesy. If your coverage requires you to have an assigned primary care provider, please ensure you are assigned to SCHC. Please remember to bring your insurance ID card and any co-payments to every appointment.
Don’t Have Insurance?
Depending on your income you may qualify for a sliding fee discount. Ask about the sliding fee discount when you call for an appointment. These discounts are due to federal funding that SCHC gets to provide discounted services for uninsured and underinsured patients.
We also offer a FREE service to individuals and families with enrolling into Covered California, Medi-Cal, and Path to Health. What is SCHC’s Sliding Fee Discount Program?
The Sliding Fee Scale is a discount program that SCHC offers through a federal grant for our patients who demonstrate financial need. The inability to pay shouldn’t be a barrier for receiving health care. Your income is vital to ensure that you pay the lowest possible price for our services. Completing our Sliding Fee Application is where we capture your household income and family size and we can alert your chart to receive any sliding fee discounts you may qualify for.
What are the benefits of the Sliding Fee Discount Program?
The Sliding Fee Scale applies to most services and can lower the cost of medical payments, lab work, x-rays, and prescriptions.
I have health insurance; can I still apply for the sliding scale?
Yes. If you qualify, the Sliding Fee Discount will be applied to any balance that insurance doesn’t cover for most services rendered at SCHC. All patients, whether insured or uninsured, are eligible.
Who pays for the services that are discounted?
The Sliding Fee Discount Program is made possible by federal grants through the Bureau of Primary Health Care.
