What is a Patient Portal?
A patient portal is a secure online website that you can access from your cellphone, tablet, or computer where you can view your personal health information and communicate with your provider.
What Can I Do Online?
- Request appointments.
- View your lab results and medical records.
- Communicate with your provider.
- Add appointment reminders to your cell phone and email.
- Access health education through the patient library.
- Make sure your information is up-to-date and complete any forms to reduce your wait time at our center.
How Do I Sign Up?
- Active users on our current patient portal will automatically receive instructions to create a new account sent to your email address on file.

COVID-19 Vaccines Coming Soon to SCHC Patients!
We continue to plan on the arrival of enough vaccine to begin an outreach to our own SCHC patients who qualify for the vaccine. At this time, we do encourage patients who are in the Public Health Tier System to go to Shasta Ready to submit your name to get a vaccine. Please be on the lookout for messages from SCHC as to our plans to get the vaccine out to our own patients.
