What are the patient surveys?

  • You may receive a mail-in survey to your home from Partnership Health Plan of California (PHC). 
  • PHC is responsible for coverage under Medi-Cal. Each year PHC works with a company to do mail-in surveys for patients of SCHC about the quality of care we give our patients. 

Why should I take this survey?

  • We use the information from this survey, along with other feedback, to make improvements to our services when we can. 
  • We also like to hear from our patients who are pleased with our services. 
  • For patients who receive this survey, please fill it out and send it in.

Please look for the survey that should already be with you or on its way very soon!

Oral Health Month 

Special Oral Hygiene Program 

Requirements

  • Far Northern Regional Center patient, where you have a service coordinator and documented diagnosis.

Program Services

  • This program is for patients with disabilities of all ages.
  • We provide patients with a dental assistant and hygienist who have specialized training for desensitizing visits.
  • Enclosed rooms for the patient to help remove distractions.
  • If needed we help schedule hospital surgeries, treatment plans, and cleanings. We can also request sedation for ages 13 and older from their primary care provider and provide shorter visits based on needs. 
  • The caregiver or parent may accompany the patient to participate in the appointment. 

Where? 

  • Redding Dental Center at 1400 Market Street Room 8103 only.

How to Sign-Up? 

  • Call our dental center at (530) 246-5700 and ask about services offered for Far Northern Regional Center patients.
  • ADULT PATIENTS: Patients who meet the requirements for this program will be exempt from the adult new patient waiting list.

HOPE Dental Program

  • Our Health Outreach for People Everywhere program provides dental services to homeless patients or those in a recovery center.
  • Located at Redding Dental Center, 1400 Market Street Room 8103 on Tuesday morning 8:00 AM to 10:00 AM only. Our doors open at 7:45 AM. Services include continuing care and urgent needs that are within our scope.
