We are excited to announce that all three of our residency program graduates will be staying on at SCHC, to continue serving our community!

What is a Patient Centered Medical Home?
A Patient Centered Medical Home (PCMH) also called a health home, is the idea that you have one place to go for most of your health care needs. It's a way for you to get better, more personal care.
What does PCMH Mean for You?
In a health home you are a part of a team that cares. You will work together to make decisions about your health and your team will also connect you with resources.
Who is Part of Your PCMH Team?
- You and those you trust
- Your primary care clinician
- Your nurse
- Your pharmacist
- Health educators
- Care navigators
Before Your Visit
- Write down your symptoms and questions for your clinicians.
- Bring all your medications with you to your appointment.
- Ask someone you trust to come with you. They can help you keep track of important details and support you.
During Your Visit
You are assigned one clinician to be in charge of your care. If you are in need of an interpreter, let us know before your visit so that we can arrange this for you. So your clinician can focus on you, a scribe will take notes during your visit.
What You Need to Do
- If there is something you don't understand, ask questions.
- At the end of your visit, ask for details in writing. For example, prescriptions, lab tests, your care plan, and date of your next visit.
