What is a Patient Portal?

A patient portal is a secure online website that you can access from your cellphone, tablet, or computer where you can view your personal health information and communicate with your provider. 

What Can I Do Online?

  • Request appointments.
  • View your lab results and medical records.
  • Communicate with your provider. 
  • Add appointment reminders to your cell phone and email. 
  • Access health education through the patient library. 
  • Make sure your information is up-to-date and complete any forms to reduce your wait time at our center.

What Should I Know?

  • The new patient portal will launch in the beginning of March
  • Active users on our current patient portal will automatically receive instructions to create a new account sent to your email address on file. 
  • For those who are not current users of our patient portal - stay on the lookout for next month’s newsletter. We will have a self-registration link that will let you sign up for the new patient portal.

COVID-19 Vaccines

At SCHC, we do not have a timeline yet as to when we will begin vaccinating our patients. As soon as we do we will let you know. 

To find out more information about what groups are being vaccinated or the location of vaccination sites go to Shasta Ready’s COVID-19 Vaccinations page. On this page you will be able to: 

  • Complete a form that will notify you by email when you are eligible for a COVID-19 vaccine. 
  • View up-to-date information on groups being vaccinated now. 
  • Make an appointment to be vaccinated if you fall   within the groups who are being vaccinated. 
  • View future dates and times for public health mass  vaccination clinics. 
  • Complete pre-registration paperwork and consent forms.
