If you have a complaint, please talk with your doctor or health plan representative.

- If problem is not resolved -

File a complaint with your plan
Call your health plan. File a complaint over the phone or in writing. Your plan responds with a decision within 30 days, or, within three days for urgent problems.

- If problem is not resolved -

File a complaint with the state
Call the HMO Help Center at 1-888-466-2219, or,
Call for a Medi-Cal State Hearing at 1-800-952-5253, or
Fill out an online complaint form at https://www.opa.ca.gov/filingcomplaints/Pages/default.aspx
