Patient-Centered Medical Home Recognition

NCQA Patient-Centered Medical Home (PCMH) recognition is the most widely adopted model for transforming primary care practices into medical homes. Research confirms medical homes can lead to higher quality care and lower costs, and can improve patients’ and providers’ experience of care. The NCQA's Behavioral Health Distinction program recognizes healthcare organizations that integrate behavioral health services into primary care settings. Achieving this distinction signifies a commitment to comprehensive, patient-centered care, addressing both physical and behavioral health needs under one roof.  Our recognition highlights our commitment to providing holistic healthcare, improving patient outcomes, and enhancing overall coordination of care.

We have received the highest level of recognition for these health centers:

A CaliforniaHealth+ Community Health Center

SCHC is part of a large family of community health centers who are unified around community driven, culturally-sensitive, and patient-centered care. This is demonstrated by the "plus" we offer through high quality, integrated, team-based care under one roof and in one system. We are very proud to commit to this goal for the benefit of our patients and our community. Those of us who make this commitment are part of this growing family of community clinics and health centers in California who are recognized by the logo - CaliforniaHealth+.

Shasta Health Rock Stars

The Shasta Health Assessment and Redesign Collaborative (SHARC) is a dynamic group of health care leaders that has been working to improve access to quality affordable healthcare in the North State since 2009. The collaborative is facilitated by the Health Alliance of Northern California (HANC). SHARC’s new Shasta Health Rock Stars program seeks to increase visibility of health career opportunities for area youth and recognize local health care professionals who have devoted their careers to service of Shasta County’s underserved populations.

We are honored that several of our staff at SCHC have been recognized as Shasta Health Rock Stars.
